Main Medical Card forms and what they do

Each form may ask for proof of your address, income, savings, rent or medical reports.

MC1. Medical Card and GP Visit Card application form Main paper application form if you cannot use the online system. Review or renewal form sent in the post Lets you renew or review your existing Medical Card or GP Visit Card when the HSE checks if you still qualify People who already have a card and receive a review or renewal letter asking for updated details Use the form included with your review letter and return it to the address given on the form.
