Apply for a replacement Senior SmartPass card for Northern Ireland (FTNI2)
If you are entitled to Free Travel in Ireland, this will be shown on your Public Services Card (PSC). Your PSC with Free Travel can be used to travel to and from Northern Ireland. For travel within Northern Ireland, you use a separate card called a Senior SmartPass.

Use this topic if you or someone you support:
- Already has a Senior SmartPass card for travel within Northern Ireland
- Needs a replacement card because it has been lost, stolen, damaged, or
- Needs a new card because your name has changed and you now use a different name

The FTNI2 form has one main part plus a Confirmation section and a Declaration section.

1. Fill in Part 1 . Your details
Part 1 is about you, the person who holds the Senior SmartPass card.
Fill in each question carefully, using capital letters and putting one character in each box.
Question 1. Your PPS number
- Carefully copy your PPS number from your Public Services Card, payment letter or Revenue document
- Write one number per box
Question 2. Title
- Put an X in the box for Mr, Mrs, Ms, or
- Write another title in the “Other” box if you use a different title
Question 3. Surname
- Write your current family name in capital letters
- Use one letter per box
Question 4. First names
- Write your first name and any middle names as they appear on your birth certificate or passport
- Use capital letters
Note if your name has changed
If you are submitting this form because your name has changed, the form asks you to send copies of official documents to prove the new name.
Examples. marriage certificate, Public Services Card (PSC), driving licence.
Question 5. Your date of birth
- Fill in the day, month and year in the small boxes
- Example.

Question 6. Senior SmartPass number, if known
- If you still have any record of your card, write the Senior SmartPass number here
- If you do not know it, you can leave this blank
Question 7. Your address
- Write your home address in Ireland, one line per row
- Include your County and Eircode if you know it
Question 8. Your telephone number
- Give a phone number where the department or SmartPass office can contact you or leave a message
- Include the area code for landlines
Question 9. Your email address
- If you have email, write it clearly in the boxes
- If you do not use email, leave this blank
Before you move on, check that all relevant boxes in Part 1 are filled in correctly.
2. Fill in the Confirmation section
The Confirmation section asks you to say why you need a replacement card and to confirm that you understand what happens to your old card.
Tick the option that applies.
Confirm that your Senior SmartPass card has:
- Been lost, or
- Been stolen, or
- Been damaged
Or confirm that:
- Your name has changed, in which case you must attach copies of the relevant documents. For example: a marriage certificate, PSC, or driving licence.
Also confirm that:
- Your original card will be electronically switched off and will no longer work
- Any tickets you buy while you are waiting for a new card are not refundable
There are boxes beside this statement for Yes or No. Tick Yes once you have read and understood this.
3. Sign the Declaration section
The Declaration is a legal statement about the information you have given on the form.
By signing this Declaration, you confirm that:
- The information you give is true and complete.
- You are legally resident and living permanently in the State.
- You will tell the department about any change in your circumstances that may affect your entitlement.
You must:
- Sign your name, or make a mark if you are unable to sign.
- Write the date.
- Ask a witness to sign and date if you cannot sign yourself.
Make sure the Declaration is signed and dated before you send the form.
Where to send the completed form
Before you post
When all relevant parts are filled in, quickly check that:
- Every question that applies to you has been answered
- You have included any name-change documents if your name has changed.
- You have signed the Declaration section
- A witness has signed if you needed a witness.
- You have kept a copy or clear photos of the form, if you can
If an official reply envelope came with the form, use that envelope.
